I, the undersigned, am the legal owner (or authorized agent of the owner) of the dog named on this application. In consideration of Camp Pawsome, LLC ("Camp Pawsome") accepting my dog into its adventure-based daycare, hiking, and boarding programs, I acknowledge and agree to the following: 1. Inherent Risks. I understand that Camp Pawsome's services involve group off-leash and on-leash activities, hiking on natural trails, transportation in Camp Pawsome vehicles, exposure to weather, water, uneven terrain, insects, and interaction with other dogs and people. These activities carry inherent risks including, but not limited to, dog-to-dog bites, scratches, sprains, strains, heat or cold exposure, dehydration, tick or insect-borne illness, water hazards, transportation accidents, slips and falls, and other injuries to my dog. I understand that even well-socialized dogs can behave unpredictably, and that Camp Pawsome cannot guarantee my dog's safety or health. 2. Assumption of Risk. I voluntarily assume all risks associated with my dog's participation in Camp Pawsome's programs, including the risk of injury, illness, disability, or death to my dog, and the risk of loss of or damage to my dog's personal property (collars, leashes, harnesses, e-collars, and other equipment). 3. Veterinary Care and Financial Responsibility. I understand that Camp Pawsome may, at its sole discretion, seek veterinary care for my dog if Camp Pawsome determines it is necessary. I agree to pay all veterinary bills incurred on behalf of my dog at the time of drop-off or upon receipt of an invoice. I am financially responsible for any injury, illness, or damage caused by my dog to any other dog, person, or property during my dog's participation. 4. Owner Certifications. I certify that my dog is in good health, is current on rabies, DHLPP, and Bordetella vaccinations, is free from contagious illness, is not food- or toy-possessive, and has no history of aggressive or harmful behavior toward any person or animal. I agree to disclose any behavioral, medical, or temperament concerns to Camp Pawsome in advance. 5. Release and Hold Harmless. I hereby release, indemnify, and hold harmless Camp Pawsome, LLC, its owners, employees, trainers, volunteers, and agents from any and all claims, damages, losses, or expenses (including veterinary costs and attorney's fees) arising out of or related to my dog's participation in Camp Pawsome's programs, except to the extent caused by Camp Pawsome's gross negligence or willful misconduct. 6. Removal. I understand that Camp Pawsome reserves the right to decline or remove my dog from any activity, or to restrict future participation, if Camp Pawsome determines that my dog's health, behavior, or temperament poses a risk to the dog, other dogs, staff, or the safe operation of the program. I have read and understand this waiver, and I sign it freely and voluntarily.
